in accordance with California AB-506 persons 18 years and older must HAVE A LIVE SCAN PERFORMED TO ATTEND A YOUTH EVENT WHERE MINORS ARE AWAY FROM THEIR PARENTS / LEGAL GUARDIANS.

STEP 1:
Fill out & submit request for live scan service form.
(Instructions on page one. if on mobile through IG you will want to open this page in browser and rotate mobile device to landscape to fill out form)
after you submit you will RECEIVE an email to confirm your submission.

Step 2:
you will RECEIVE an email with a link to "Open agreement." download and print completed filled request for live scan service form.

Step 3:
sign request for live scan service.

Step 4:
PRINT THE filled & signed request for live scan service form AND TAKKE IT to a LIVE SCAN FINGERPRINTING SERVICE PROVIDER. CLICK HERE FOR THE CA DOJ DATABASE OF LOCATIONS THAT CAN PERFORM THIS SERVICE.
